Patient and Family Engagement: Tools for Implementation and Success

Friday October 24th, 2014

8:30 AM – 3:30 PM

The Capitol Center for the Arts

44 South Main Street, Concord, NH

The Foundation for Healthy Communities has planned this state-wide workshop to bring NH hospitals together with members of their communities who are willing and well-motivated to serve as patient advisors. It is our aim to ensure that the benefits from a health care culture that fully engages patients and families are realized across our State. Many Hospitals have started down this path and this workshop will add to their momentum by:

  • Demonstrating the power in each patient advisor’s personal story and life experience
  • Sensitizing hospital leaders to the contributions already being made to health care organizations by effective patient advisors and patient /family advisory councils.
  • Sharing perspectives of national leaders in the development of a progressive culture of patient engagement and the role of the Patient and Family Advisory Council.
  • Exposing all attendees to progress made by their peers across the State
  • Accelerate the readiness of patient advisors and their partners in Hospitals.

Nationally recognized subject matter experts from the Institute for Healthcare Improvement, Baystate Health and Baystate Medical Center, Dartmouth Hitchcock and resident expert Tanya Lord PhD, MPH will be our keynote speakers and facilitators. The agenda will offer ample time for interaction with faculty and with colleagues from around the State.

Patient and family advisors and those who may wish to become a patient advisor, Senior Leadership, Clinicians (nurses, physicians and others), patient experience / satisfaction staff, quality improvement staff from hospitals, medical practices /clinics, home care, and long-term care.
