Leadership Team

Granite State Health Care Coalition Leadership Team

The Granite State Health Care Coalition Leadership Team has been assembled and tasked with providing guidance and subject matter expertise in decisions regarding Granite State Health Care Coalition priorities and objectives.  

GSHCC Leadership Team representatives serve as active members of the coalition and consists of a diverse cross section of healthcare partners to ensure one sector doesn’t influence the overall objectives of the group.

 

Leadership Team Composition includes:

  • Hospitals (Large, Critical Access, Specialty)
  • Emergency Medical Services
  • Emergency Management
  • Public Health Agencies
  • Nursing Homes
  • Home Care Agencies
  • Federally Qualified Health Center/Community Health Center Association

 

Administrative Lead Organization

The Foundation for Healthy Communities (FHC) has been contracted by NH DHHS to be the Administrative Lead Organization for the GSHCC. Current funding comes from ASPR under the HPP and PHEP Cooperative Agreement. Duties of the Administrative Lead Organization include but are not limited to: assistance with recruiting and orienting new GSHCC members; ensuring effective communication and coordination between and among members; providing and conducting training and educational opportunities; and serving as the administrative and fiscal agent of the GSHCC grant. The Foundation for Healthy Communities is responsible for staffing and the oversight of any subcontractors.
